Milestones participates in most of the major insurance plans including Medicare, Medicaid, Anthem Blue Shield, Magellan, and SIHO. Before you make your appointment, we suggest you contact your insurance carrier to verify if there are any referral, pre-certification, or network participation requirements on your policy.
At your first appointment you will be asked to provide us with information about your insurance - please bring your card with you. In most cases we will file a claim to your insurance on your behalf.
We request and appreciate payment at the time of service for that portion of the fee for which you are responsible (e.g. co-pays, deductibles or the entire fee). Statements are mailed monthly. We accept Visa, MasterCard, and Discover for your convenience.
On subsequent visits to our office, you will need to inform our staff of any changes in your address, phone numbers, or insurance information.
